Below are the 4 documents (Participant Release, Youth Covenant, Americana Community Center Release and St. Joseph Children's Home Release) that I need from each of the students going on the mission trip! If you do not have access to print them off we would be more than happy to provide you with physical copies!
Students will not be able to attend unless I have received these documents from them!
I would like to have them back ASAP, but the deadline for them to be turned in is May 19th.
